Community Care Coordination

Challenge

Within the healthcare industry, 20% of the population is responsible for 80% of the costs.  Within behavioral health, this 20% typically have a severe mental illness diagnosis and a history of poor treatment response due to stigmatization of illness, co-morbid conditions, inconsistency of medication adherence, and lack of adequate support networks.

In addition the fundamental barriers of fragmentation of healthcare services, focus on prescriptive versus preventative medicine, and inadequate utilization of resources have contributed to ongoing behavioral health disturbance and increased utilization of multiple healthcare resources.

Our Solution

Community Care Coordination is designed to address these fundamental barriers.  Our goal is to go beyond fragmented treatment and engage participants and providers to work together towards a common Care Plan.

Community Care Coordination programs are developed based upon the requirements of the purchaser and needs of the individual.  The approach is consumer centered and focuses on

• Engagement of consumer through face-to-face contact at home, clinics, or facilities
• Identifying root causes for non-adherence through a comprehensive bio-behavioral assessment
• Coordination of services through a care plan that is shared with primary care and other specialty care providers
• Monitoring medication adherence
• Development of community resources and support systems

Community Care Coordination is a Treatment Team Approach.  In addition to their community healthcare providers, each participant has twenty-four hour access to:

• A CBHM Psychiatrist - Reviews diagnoses and treatment plan
• Care Coordinator - Is the Team Leader who provides face-to-face contact and coordinates Care Plan with all healthcare providers
• Behavioral Health Specialist - Assists with basic resource development (housing, transportation) and scheduling appointments
